Case management programs assure that emotionally disturbed children and their families have access to the services and supports necessary to help them function in the community.  Case managers in the Children's Mental Health System endeavor to assist children with serious emotional difficulties to remain within their own homes, schools and communities by arranging for an array of services, developing strong linkages among the systems serving children and families, advocacy efforts and individualizing service plans to meet the specific needs of children and families.

Project serving Mount Vernon, Yonkers, New Rochelle, Peekskill and Ossining.  Mount Vernon site in collaboration with the Westchester County Youth Bureau.  Program provides assessment and a full array of treatment and support services, either directly or through collaborating agencies.  Services include screening and assessment, clinical services, case management, in-home services, rehabilitation, recreation, family support, self-help initiatives, transportation and emergency funds.  Individualized care plans are developed, and the program works with one or more child-serving systems (education, child welfare or juvenile justice).  Flexible services respond to individual needs and are available at convenient times and places.  Staff is available 24 hours a day for crisis situations.  Bilingual staff is available for SED children and their families, with priority for those identified by Networks, local groups which bring together numerous service systems to help with individual children and families.

Project works with TANF families to enable parents to work while meeting the needs of their SED children.  Services include case management, treatment, job placement through a vocational counselor and support services collaborative program with Westchester County Department of Social Services.  Connection to active community planning group.

Children's Intensive Case Management

Twenty-four hour, intensive services and limited financial assistance provided to children and youth with SED who have been hospitalized in the past, and who are at risk for re-hospitalization.  For children ages 5 to 18.

Partnership Project

Coordinated multi-system approach including Department of Social Services, Youth Bureau and Mental Health.  Provides intensive, individualized services to families with at least one child with emotional disturbance at risk of residential placement.  Includes limited financial assistance and priority access to services in a variety of systems.  Priority access to services through Children and Family Networks in Mount Vernon, New Rochelle, Port Chester, Central Westchester, Mount Kisco and Bedford, Ossining, and Peekskill.

Intensive 24-hour individualized services to families with a child at risk of placement in a residential treatment facility.  Utilizes a strength-based approach in the youngster's home community with a wide variety of services including crisis response, intensive in-home services, skill-building, respite and family support.

Assertive community mental health treatment for adolescents and young adults from 17 to 21 years of age who have a history of serious emotional problems.  Provides outreach; case management; individual, group and family mental health treatment; medication management; crisis intervention; psychoeducation; skill development and enhancement; assertiveness and problem solving strategies; advocacy; and access to educational and vocational training and related services.  Intensive Case Management For Aging-Out Youth.  Twenty-four hour, intensive services for SED older adolescents and young adults who have been receiving educational services outside their school districts or outside the state.  Referrals made by local Committees on Special Education.
