CPP Families in Transition

Center For Preventive Psychiatry – Families in Transition (FIT) is a new initiative designed to facilitate the transition of Yonkers (and soon to include Mount Vernon) families from welfare to work.

Program Services:  FIT uses an intensive clinical and case management approach to assist families in overcoming barriers to work.  Families are helped to identify strengths and to utilize community supports as they make this transition.  FIT families will have access to a full array of mental health and case management service, in addition, the Director of Employment Services assists head of households to analyze their skills and locate appropriate job opportunities.

Population Targeted: 

• Families must be currently be receiving public assistance – TANF (Temporary Assistance for Needy Families).
• Must currently have an open case with Department of Social Services.
• Must have at least one child in the family who is experiencing difficulties in school or at home.
• Head of household must be ready for transition to work.
